From wellness app to integrated care: How Sirka is building an AI-enabled metabolic healthcare model

ACV Capital-backed Sirka combines physical healthcare expansion with AI-enabled clinical workflows to address Indonesia’s underserved metabolic healthcare market.

Indonesia has a large and underserved market for metabolic healthcare. An estimated 93 million adults are overweight or obese, while access to supervised metabolic care remains limited relative to the potential patient population.

“Most people who need metabolic care in Indonesia never reach the doctor. They drop out before the first conversation starts,” said Rifanditto Adhikara, Founder and CEO of Sirka.

Founded in 2021, Sirka initially launched as a wellness app focused on diet coaching and professional consultations. It later expanded through partnerships with healthcare clinics before opening its first physical clinic in 2024.

The business has since grown to two clinics in Greater Jakarta, focused on weight management, digestive health, diabetes care and chronic disease management, with three additional locations planned. The company was named to Forbes Asia 100 to Watch 2026 in the Biotechnology & Healthcare category.

Beyond the initial consultation

Sirka has expanded beyond its initial wellness offering to include physician-led care, diagnostics, pharmacy services and structured health programmes.

The model supports patients through more of the care journey, from initial consultation and diagnosis to medication and ongoing care. Bringing these services together also gives the company a longer view of treatment outcomes and adherence.

The expansion into these services has also diversified Sirka’s revenue streams. The business has reached profitability.

AI nearly doubles conversion while cutting physician time

Sirka’s use of AI comes as consumers become more comfortable using the technology for health-related needs. A 2026 BCG study of more than 13,000 consumers across 15 countries found that nearly 60% use generative AI for personal health, although human involvement remains important in healthcare.

At Sirka, AI supports different stages of the patient journey. Its AI Receptionist handles intake, triage and risk routing, while Doctor AI reviews patient information and prepares a structured pre-analysis ahead of the consultation.

One of the first areas of focus was the time patients spent waiting to reach a doctor. Manual intake, triage and routing accounted for most of the approximately 26-minute wait before a consultation.

With more of these steps handled by AI, median consultation start times fell from 26 minutes to three minutes.

The shorter waiting time was expected. The effect on conversion was not. Among newly acquired patients in the pilot, conversion increased from 33% to 60%.

“What surprised us more was the conversion that nearly doubled. That was the true signal that, for us, AI was truly creating value,” Rifanditto said.

Physician time per patient also declined. With patient information analysed before the consultation, the average fell from 18 minutes to eight minutes, allowing physicians to spend less time gathering information and more time reviewing each patient’s case.

Building across digital and physical care

Sirka plans to add three clinics to its existing two locations in Greater Jakarta, while continuing to develop its digital care model.

The new locations will expand access to Sirka’s metabolic healthcare services as the company grows.
